Case 2024: Dominik (expert opinion by J. Stranne)

Dominik, 60 years old, was born and raised in Vienna. He is a big fan of the local cuisine and his favourite dish is the Wurstgulasch. Dominik always orders it when he goes to a local restaurant with his wife. Recently, he wanted to surprise her by cooking it himself, after which she immediately called the restaurant to book a table for their next night out.

Unfortunately, he had a PSA of 14.8 ng/ml during a check-up at the GP. He was referred to a urologist. The initial work-up resulted in the following:

  • DRE: cT2b
  • Prostate volume: 35 ml
  • MRI: PI-RADS 4
  • Prostate biopsy: ISUP grade 4 (Gleason 4+4) (2/3 targeted and 2/12 systematic)
  • PSMA-PET/CT: N0M0

He is otherwise healthy and has no major comorbidities.

Which option would you suggest for Dominik?
