Case 2025: Manuel (expert opinion by V. Achard)

Manuel, 74 years old, inherited his love for the Portuguese guitar from his grandfather, a master of the instrument. A naturally melancholic soul, he turned his passion into a profession, filling the local tavern with the soulful sounds of Fado every evening.

Consider this patient’s medical history:

  • 5 years ago: radical prostatectomy for T2N0M0 PCa (ISUP grade group 3 (Gleason 4+3))
  • 2 years ago: early prostate bed salvage RT without ADT for BCR (PSA: 0.22 ng/ml)

Follow-up information today:

  • PSA: 1.2 ng/ml
  • PSA-DT: 7 months
  • Asymptomatic, no relevant comorbidities
  • ECOG PS: 1
  • PSMA-PET/CT: uptake in right acetabulum and L1 spinous process with no morphologic lesion on the CT

Which option would you choose for Manuel (not taking into account regulatory approval and local restrictions)?
